New Customers

New users must first fill out the Customer Setup Form.

Sample Requests

After that is complete, the Sample Request Form must be filled out and the corresponding Sample Request Form Excel File (XLS, 16KB) must be emailed to .

Analytical Requests

If further analytical work is needed (i.e., any procedure on the analytical procedures page), the Analysis Request Form must be filled out and the corresponding Analysis Request Form Excel File (XLS, 29KB) must be emailed to .
